Program/Service Notes:

MSSP is an elderly independence program, funded through State and Federal Agencies. MSSP provides a caring and qualified Care Manager who will meet with you to learn your needs for independent home living. Your care manager will arrange for the assistance that you need and will check regularly to make sure these services continue to meet your needs. Services include: meal prep and delivery; emergency alert device; help with governmental or insurance paperwork; physical and occupational therapy; transportation; protective supervision; nursing care; support groups; respite; personal care; home repairs & safety equipment; legal advice; friendly visitor; psychological counseling; house cleaning; shopping; medical monitoring; low-cost housing.
Hours:	8 am to 5 pm, Monday through Friday
Eligibility:	65years +;Medi-Cal recipient;Frail health; desire to live at home; willing to work with MSSP staff on your own behalf; resident of Siskiyou, Shasta, Lassen, Trinity Counties.
